FIRST MEETING
Your first session will consist of an intake/assessment and will take 60 minutes. Subsequent meetings will be 45-53 minutes, depending on insurance rules.
INSURANCE INFORMATION
606 Wellness, LLC is an in-network provider with Blue Cross Blue Shield PPO and Cigna. I am an Out-of-Network provider for most other insurance plans. Self-pay clients are also accepted. If you have insurance with another company, I can provide a statement for you to submit for possible partial reimbursement through your out-of-network benefits. Payment is due at the time of services and may be made via cash, check, credit card, Venmo, and Zelle.
You are strongly encouraged to contact your insurance carrier before your session to understand any co-payments, co-insurance, or costs for which you may be responsible. Services may be covered in full or part by your plan. You may check your coverage by calling the customer service number on the back of your insurance card.
